Your best bet is probably going to be having it inspected and or appraised by a registered gemologist or other associated professional who's been trained in such matters. There are some other tests you can do on your own that may help also, but a second opinion never hurts.
There are multiple factors that go into determining if a stone is a real mined stone, a lab created stone (this will cost less, but are 99.9% identical in every way to an actual mined stone), to a fake/forgery. Generally how light reacts with the stone, weight, heat, even down to the quality of the setting if the stone is already in a piece of jewelry.
